ATHLETIC PARTICIPATION INFORMATION
2021-2022 UPDATED Participation Forms: These forms have been updated per the KHSAA. These are the only forms that will be accepted for any physical examination that is administered on or after June 1, 2021. There will be no exceptions. Please print this packet, complete all required parent/student sections before your physical examination, then submit the entire packet to the Athletic Department after the doctor has completed physical evaluation sections. Should you have any questions, please contact the Athletic Department.
Deadline for Student-Athlete Participation Forms
In order for student-athletes to participate in anything athletics, they must have the current year's paperwork turned into the Athletic Office. New school year, means new paperwork. If the physical is still current, the remainder of the packet (7 pages) needs to be complete before any preseason workouts. This includes open gyms/fields, lifting, or any conditioning.

The information in the above Participation Handbook is very important. Please take the time to read it carefully! Participation in the Saint Xavier Athletic Program is a privilege, not a right. Upon making the decision to participate, student athletes and parents must be prepared to make commitments. In addition, participation requires student athletes and their parents to accept and abide by the policies and procedures detailed in this handbook and as required by Saint Xavier High School in its rules and codes for all students, as well as all policies and bylaws contained in the handbook of the Kentucky High School Athletic Association.
One of the most important documents for Saint Xavier student athletes, the Parental Consent/Permission to Treat/Physical Examination Form, must be completed and returned to the Saint Xavier Athletic Office before any student athlete will be permitted to participate in any aspect of the athletic program. KHSAA rules and insurance requirements dictate that student athletes and parents complete this form and that there are no exceptions to this policy.
